A Patient Centered Medical Home is a model of health care where the care is “centered” around your needs; providing
  •  same-day appointment
  •  information regarding a particular disease or diagnosis
  •  phone calls returned on a timely basis. 
  • coordination of your care with all your providers
  • preventive services to maintain your health
  • patient engagement outside of your visit
Your doctor leads a team of health care professionals that collectively provide medical needs – both in wellness and in illness.
What is your role?
  • actively participate in your care
  • ask questions and share information
  • prepare for your visits- bring your medications, immunization records, personal health records- to help your care team provide the most care to you
  • provide us a list of all your physicians and providers
  • inform us when you visit specialists
  • inform us when you are hospitalized or visit the emergency room

Primary Care First
Alliance Primary Care of New Jersey’s Participation in  
Primary Care First (PCF) Is Ending 

The Centers for Medicare & Medicaid Services (CMS) will be ending the Primary Care First (PCF) Model for all participating practices, effective December 31, 2025. All Beneficiary Engagement Incentive(s) and any Benefit Enhancements associated with the PCF Model will also end on this date. We want to assure you that, despite this change, the quality of care you receive at our practice will remain consistent and unchanged.  

Please be aware that the conclusion of our participation in the PCF Model does not change your Medicare benefits. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it is always your choice which doctor you use or hospital you visit. You continue to have the right to use or visit any doctor or hospital that accepts Medicare, at any time.

Dedicated Care coordinator to help our patients with complex needs navigate their health care.
Patient Family Advisory Council
In an effort to engage our patients in their own care, we have formed a Patient Family Advisory Council (PFAC).

Behavioral Health Care
We work with our partners to provide care management and access to Behavioral Health services.
